Requesting a User Account 
All full-time and part-time faculty and staff employed by the College will automatically be provided a network, Blackboard, myFSC (portal) and E-mail account after they have been entered into the College's Payroll System.
Facilities staff, consultants, non-payroll contractors, and third party service providers require department head or area Vice President authorization, and the approval of the Chief Information Technology Officer (or his designee), in order to receive an E-mail account. In order to ensure that access privileges are properly authorized for these individuals, and modified when affiliation with the College changes, an “ Application for Access to Information Systems and Services ” must be submitted for approval prior to a user account being activated. Each application for access privileges must be authorized by the area Vice President or department head before submitting the request to ITS-User Services for Chief Information Technology Officer approval.
Each individual must agree to the College's “ Acceptable Use Policy ” before accessing the campus computing network.
Since we require a signature from your supervisor/chair, a hardcopy of this form must be submitted.
We require at least one business day to complete the request once we receive the form.
